WHAT: A new Coast Guard report blasted Transocean for the major role that its flawed equipment, sloppy maintenance and inadequate emergency training played in contributing to the deadly Deepwater Horizon drilling rig explosion that set off the Gulf of Mexico oil spill disaster.

WHY: As owner of the drilling rig, Transocean was primarily responsible for ensuring its safety. But gas alarms and automatic shutdown systems failed to alert the crew. And workers didn’t receive proper training on disconnecting the rig from the well to avoid an explosion.

WHO: The Coast Guard was especially harsh with the Marshall Islands, a tiny Pacific nation where Transocean registered the Deepwater Horizon in 2005. The report said the Marshall Islands “abdicated its vessel inspection responsibilities” by farming out the work to private companies.
